Facial Plastic Surgery Questions and Answers: Part 04

Question: I’m turning 50, is it time for a facelift?
Answer: The Photographs demonstrate early onset jowling, and inelastic skin in the neck. A lower face and neck lift can accomplish improvement by tightening loose facial and neck skin, tightening loose facial and neck muscles, lifting the jowls and removing a small amount of fat present in the neck itself with liposuction.

Question: Would I be able to have a closed rhinoplasty to fix the asymmetry in my nose?
Answer: Yes, a closed rhinoplasty approach can accomplish fixing the asymmetries in your nose, refining the bulbous nasal tip, and narrowing the the bridge line. Question: Mini Neck lift or more ultherapy?
Answer: From the Limited photographs presented, you have fat deposits in the neck and platysmal muscle bands. A mini necklift would accomplish removal of the fat with liposuction, and tightening the platysmal bands surgically. At age 42 your skin tone should be fine and no skin removal should be necessary.

Question: Do I need brow lift or upper blepharoplasty to reduce the outer crows feet?
Answer: Crows feet are best treated with Botox, not surgery. The height of your eyebrows is acceptable, and a browlift is totally unnecessary. There is not enough extra skin on your upper eyelids, therefore, eyelid surgery is not necessary.

Question: Is my nose round? Main focus is fixing the bridge. Thoughts?
Answer: A closed rhinoplasty procedure can accomplish reduction of the bulbous tip, lifting the droopy tip, shaving down the dorsal hump and narrowing the bridge line with all of the incisions placed on the inside of the nose. No external incisions are required, and no painful packing is required either. It’s also important to release the depressor septi ligament, which dynamically pulls the tip downwards when smiling. Digital computer, imaging of your nose upon your facial features would be helpful to understand what can and cannot be accomplished.

Question: Is my chin implant too high, too small?
Answer: From the one limited photograph, it appears that your chin implant is too high, and probably not large enough since you still have a recessive chin present. You also have some fat deposits in your neck, which can be removed with liposuction. All of this can be accomplished under local anesthesia is an outpatient procedure.

Question: 45 y/o concerned about neck bands – what can I do to correct this?
Answer: The most conservative treatment for your neck bands would be to have Botox placed in the platysma muscle. If that fails, then consider a surgical platysma plasty with a myotomy.

Question: Is it possible for me to get rhinoplasty with twilight/conscious sedation? 
Answer: Twilight means you you will not have any recollection, so you’re under a very deep intravenous sedation. Conscious sedation means you’ll have conscious awareness of any type of plastic surgery. Due to the fact there’s lots of nerve endings in the nose, and it’s very difficult to get the lidocaine injections performed comfortably, in addition to osteotomies placed in the nasal bones comfortably, it’s usually best to perform this procedure under general anesthesia for patient safety, and comfort. It’s very important to have a controlled airway since rhinoplasty and septoplasty are airway procedures.

Question: What rhinoplasty procedure would be the best for my nose?
Answer: A closed rhinoplasty approach can accomplish shaving down the dorsal hump, decreasing the overall projection of the nose, reducing the bulbous nasal tip, and narrowing the ridge line with all of the incisions placed on the inside of the nose. No external incisions are required, and no painful packing is required either. Digital computer imaging would be helpful to understand what can be accomplished with your nose upon your facial features. Rhinoplasty is a very difficult endeavor, so choose your surgeon wisely based on extensive experience.

Question: Chin lipo, Kybella or mini facelift?
Answer: The photos are rather limited, especially in the different positions that the photographs were taken. If there’s a small amount of fat in the submental area, this can be removed a simple liposuction.
